Description
Arising from the ophthalmic artery near the orbital apex, paired muscular branches course anteriorly within the intraconal fat to reach the extraocular muscles and the globe, running in close company with the optic nerve and ophthalmic veins. Medial and lateral muscular arteries track along the rectus muscles, giving small arterioles that perforate the muscle bellies and send recurrent twigs proximally toward the common tendinous ring. Anteriorly, terminal contributions approach the anterior segment near the limbus in relation to the sclera, with the iris and pupil indicated as surface landmarks on the globe. Emphasis on the muscular arteries matters because they sit in the operative corridor for orbital decompression, optic nerve sheath fenestration, and strabismus surgery, where bleeding can rapidly obscure planes and complicate muscle identification. Their proximal origin near the annulus of Zinn also explains why pathology at the orbital apex, including idiopathic orbital inflammation or cavernous sinus related venous congestion affecting ophthalmic venous drainage, can present with painful ophthalmoplegia.
