Description
Arising from the ophthalmic artery as it enters the orbit through the optic canal, the muscular arteries course anteriorly within the intraconal orbital fat, tracking along the superior and inferior surfaces of the extraocular muscles. Superiorly, small branches run adjacent to the superior rectus and levator palpebrae superioris, while inferior muscular twigs pass toward the inferior rectus and inferior oblique, with collateral connections toward the lacrimal artery laterally and the medial palpebral region medially. The optic nerve lies posterior and medial to the globe at the orbital apex, and the superior orbital fissure sits posterolateral as a key gateway for accompanying neurovascular structures. Small vessels, big consequences. This superior, sectioned-skull perspective matters because the muscular branches are the workhorse supply for the recti and oblique muscles and form part of the anterior segment circulation via their anterior ciliary arteries. In thyroid eye disease, orbital decompression, or strabismus surgery, knowing where these vessels run within the cone helps you anticipate bleeding planes and avoid ischemic complications, including anterior segment ischemia when multiple rectus muscles are operated on in the same session. It also frames the relationship between vascular channels and the orbital apex, where crowding can threaten optic nerve perfusion.
