Description
Arising from the internal carotid artery at the level of the optic canal, the ophthalmic artery is traced from a superior vantage as it turns anteriorly into the orbit, running in close relationship to the optic nerve and then crossing toward the medial orbital wall. A coronal section of the adult male skull frames the paired bony orbits, with the globes seated anteriorly and the optic nerves (cranial nerve II) coursing posteriorly toward the chiasmatic region. Red arterial channels and blue venous pathways outline the vascular envelopes of the orbit while blue-coded cranial nerves traverse the superior orbital fissure region in parallel but distinct trajectories. Spatially, the ophthalmic artery sits superior to the optic nerve early, then shifts medially as it advances. That superiorly oriented course is not an academic detail. During endoscopic endonasal and orbital apex procedures, the artery’s relationship to the optic nerve and optic canal sets the risk profile for iatrogenic visual loss, and it also explains why an orbital apex hematoma can compress both the vessel and nerve in a confined space. Clinically, ophthalmic artery occlusion and embolic events after carotid disease or facial filler injection follow this arterial route, with retrograde flow through anastomoses to the external carotid system providing a plausible mechanism for sudden blindness.
