Description
Presented in an anterior (palmar) orientation, the carpus is centered with the radius and ulna proximally and the metacarpal bases distally, while the scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate are bound by the palmar intercarpal ligaments spanning adjacent rows. Short, obliquely oriented fibers bridge the intercarpal intervals, with thicker bands crossing from the distal radius toward the proximal row at the radiocarpal articulation. Volarly, the ligamentous bands lie deep to where the flexor tendons and carpal tunnel contents would course, forming a stabilizing layer over the small, mobile bones. Fine detail matters here. Palmar ligament architecture becomes clinically relevant when evaluating carpal instability patterns that do not declare themselves on a simple dorsopalmar radiograph, such as scapholunate dissociation with abnormal scaphoid flexion or lunotriquetral injury with VISI mechanics.
