Description
Running proximodistally through the medial forearm, the ulna is presented in anterior (palmar) orientation from the elbow to the wrist. Proximally, the olecranon and coronoid process frame the deep trochlear notch, a C shaped concavity that faces anteriorly to receive the trochlea of the humerus, while the radial notch sits on the lateral side for the head of the radius at the proximal radioulnar joint. Distally, the shaft tapers to the ulnar head and ulnar styloid process, with the interosseous border coursing along the lateral margin for attachment of the interosseous membrane. Anterior viewing clarifies why the ulna behaves as the primary stabilizer of the elbow joint while the radius rotates around it during pronation and supination. The relationship between the coronoid process and trochlear notch is the key bony restraint against posterior elbow dislocation, and it is the segment fractured in coronoid fractures associated with the terrible triad injury (elbow dislocation, radial head fracture, coronoid fracture). A clean read of the radial notch and distal ulnar head also helps when teaching distal radioulnar joint mechanics and why ulnar variance can alter load transmission across the ulnocarpal complex.
