Description
Centered on the male pelvic brim, the right and left common iliac veins converge inferior to the aortic bifurcation region to form the inferior vena cava, with the external iliac veins tracking inferolaterally along the pelvic inlet toward the femoral veins at the level of the inguinal ligament. Medial tributaries consistent with the internal iliac veins enter the common iliac veins on each side, spreading into an anterior pelvic plexiform pattern overlying the bony pelvis. The os coxae and sacrum provide fixed landmarks, while the proximal femora frame the outflow into the lower limbs. An anterior perspective on this junction matters because the left common iliac vein is classically crossed by the right common iliac artery near the lumbosacral junction, the anatomic setup for May-Thurner syndrome with left iliofemoral deep venous thrombosis. This is also the venous crossroads encountered during anterior approaches to the lumbosacral spine and pelvic surgery, where traction or misidentification can injure thin-walled iliac veins and produce rapid hemorrhage.
