Description
Anteriorly, the superficial and deep venous pathways of the male shoulder converge toward the axilla, where the axillary vein continues proximally as the subclavian vein at the lateral border of the first rib, deep to the clavicle. Tributaries from the upper limb, including paired brachial veins and the basilic vein, join the axillary vein medial to the humerus, while the cephalic vein tracks along the deltopectoral groove to pierce the clavipectoral fascia and drain near the infraclavicular fossa. Across the superior chest, subcutaneous tributaries over the pectoralis major feed into the axillary and subclavian systems, with venous channels coursing medial to the deltoid and inferior to the clavicle. Landmarks read cleanly in this projection. Venous anatomy here matters because many high volume procedures target this corridor, and a few millimeters of anatomy changes your complication profile. Infraclavicular subclavian venous access and axillary vein cannulation for central lines or ports run adjacent to the pleural dome and first rib, while pacemaker and ICD leads traverse the cephalic, axillary, or subclavian vein and can precipitate stenosis or thrombosis of the thoracic outlet. A critical teaching point is the cephalic vein’s terminal segment at the deltopectoral triangle, a common surgical cutdown route when percutaneous access is undesirable. Variants are common.
