Description
Arising centrally at the cranial base, the sphenoid bone is presented from an inferior orientation within a transparent adult male skull, with the body of the sphenoid positioned medial to the greater wings and anterior to the basilar part of the occipital bone. Inferiorly, the medial and lateral pterygoid plates descend from the junction of the body and greater wing, separated by the pterygoid fossa and terminating in the pterygoid hamulus on the medial plate. Anterior to the pterygoid processes, the posterior nasal apertures (choanae) and vomer align along the midline, while laterally the maxillary alveolar arch and mandibular dentition provide facial reference points through the transparent craniofacial framework. For teaching skull base anatomy, this is the angle that makes the pterygoid region intelligible, because you can relate the pterygoid plates to the posterior hard palate and nasopharyngeal airway without losing the midline. Surgeons use these bony landmarks when planning approaches through the nasal cavity to the central skull base, and dentists and maxillofacial teams reference the pterygoid hamulus and adjacent pterygomaxillary region when discussing pterygoid plate fractures, posterior maxillary procedures, or spread of infection into the pterygopalatine fossa.
