Description
Seen from the inferior aspect, the orbital surface of the frontal lobes comes into view with the gyrus rectus running anteroposteriorly near the midline and the medial orbital gyrus positioned just lateral to it. Across this most medial orbitofrontal cortex, the medial orbitofrontal artery courses in a fine, arborizing pattern, consistent with a small-caliber cortical branch (arteriole-level in its distal distribution). Posterior to the orbital gyri, the optic chiasm region and adjacent basal forebrain landmarks provide orientation, while cranial nerves emerge from the ventral brainstem in close proximity. Small vessel anatomy matters here. This inferior perspective is the one you want when teaching or planning around the anterior skull base, because it ties the vascular map directly to the surface most vulnerable in frontal contusions and iatrogenic injury. The medial orbitofrontal artery typically arises from the anterior cerebral artery system (often via the A2 or pericallosal-callosomarginal region depending on nomenclature and branching pattern), and its distal field overlaps cortex involved in decision-making and olfaction-related processing. During endoscopic endonasal and transcranial approaches to the planum sphenoidale and cribriform plate, vascularized dura and small cortical branches on the orbital surface can complicate hemostasis, and infarction in this territory may contribute to postoperative personality change or abulia rather than classic motor deficits. Neuroanatomy and neurosurgery courses use this angle to connect orbitofrontal gyri, cranial nerves, and anterior cerebral circulation without flipping between textbook plates. It also fits well in atlases and journal figures discussing anterior communicating artery aneurysm surgery, subfrontal retraction risk, and basal frontal contusion patterns in traumatic brain injury.
