Arising from the anterior cerebral artery near the anterior communicating complex, the medial orbitofrontal artery courses anteriorly along the medial orbital surface of the frontal lobe, closely applied to the gyrus rectus and the medial orbital gyrus. From an inferior (basal) brain view, its fine cortical branches can be followed toward the frontal pole, running medial to the olfactory tract and bulbs and anterior to the optic chiasm in the region of the anterior perforated substance. Nearby vascular landmarks typically include the A1 and proximal A2 segments of the anterior cerebral arteries, the supraclinoid internal carotid arteries laterally, and the proximal middle cerebral arteries entering the sylvian fissures.
