Description
Viewed from an inferior aspect in an adult male, the right hepatic vein is seen coursing superiorly within the right lobe of the liver and converging toward the posterior cranial surface where it enters the inferior vena cava. Tributary channels from the deeper parenchyma join it at acute angles, reflecting drainage from the right-sided hepatic segments (classically segments V to VIII) toward a short, broad outflow tract. Medially, the middle hepatic vein territory lies closer to the main fissure, while laterally the hepatic capsule outlines the convex margin of the right lobe. Short hepatic veins to the retrohepatic inferior vena cava may be suggested posteriorly. A high-flow junction. That inferior perspective matters in hepatobiliary surgery because the right hepatic vein is a key landmark during right hepatectomy and living-donor procurement, where control of venous outflow determines congestion risk in the remnant liver. The relationship of the right hepatic vein to the retrohepatic inferior vena cava and to segmental planes also underpins Couinaud-based resection strategy and helps explain postoperative complications such as venous outflow obstruction or bleeding from short hepatic veins during caval dissection. Interventional radiologists reference the same anatomy when planning hepatic venography, TIPS variants, or troubleshooting Budd-Chiari pattern outflow compromise.
