Description
Dominating the posterolateral aspect of the right hepatic lobe, the right hepatic vein courses superiorly within the hepatic parenchyma and expands into a broad ostium at its junction with the inferior vena cava. Tributaries from the right lobe converge in a dendritic pattern, then coalesce into a single large channel as it approaches the caval groove. Superiorly, the vein meets the IVC just inferior to the diaphragm, while remaining lateral to the middle hepatic vein and medial to the bare area of the liver. Blue coding emphasizes venous outflow from the hepatic sinusoids toward the systemic circulation. Attention to the right hepatic vein is practical for hepatic resection planning because it defines anatomic territories of venous drainage and often guides the plane of a right hepatectomy. In living donor liver transplantation, preserving adequate venous outflow from the right lobe can require reconstruction when sizeable accessory right hepatic veins are present, or when segmental drainage would otherwise be compromised. Short hepatic veins entering the IVC near the right hepatic vein can also be a bleeding hazard during mobilization of the right lobe. Small structures, big consequences. Use this illustration in gross anatomy teaching for the abdomen, in hepatobiliary surgery texts that discuss inflow and outflow control, or in radiology and ultrasound instruction to correlate hepatic vein anatomy with Doppler waveforms and Budd-Chiari patterns of hepatic venous obstruction.
