Description
Posterior to the thyroid gland, the paired superior and inferior parathyroid glands sit against the thyroid capsule, usually two on each side. The superior parathyroids lie more constant in position, typically near the posterior aspect of the upper thyroid lobes around the level of the cricoid cartilage, while the inferior parathyroids tend to rest closer to the lower poles and can drift inferiorly toward the thyrothymic ligament. Medially, their relationship to the tracheoesophageal groove matters, with the recurrent laryngeal nerves ascending nearby and the esophagus lying posterior to the trachea. Small structures. Big consequences. Surgical anatomy drives demand for this view. During thyroidectomy and central neck dissection, the parathyroids must be identified and preserved to prevent postoperative hypocalcemia from inadvertent devascularization or excision, and understanding the typical superior versus inferior positions helps you anticipate ectopic glands when one is not found on the posterior thyroid surface. Their arterial supply commonly comes from the inferior thyroid artery, so ligating terminal branches close to the thyroid capsule, rather than tying the main trunk laterally, better protects parathyroid perfusion. This relationship also underpins how parathyroid adenomas may be missed in exploration when an inferior gland has migrated into the superior mediastinum.
