Description
Viewed laterally in the anterior neck, the thyroid gland occupies the pretracheal region, with its posterior surface and capsule used as the reference plane for locating the glandulae parathyroideae. The superior parathyroid glands lie more constant in position, typically posteromedial to the upper thyroid lobe near the cricothyroid junction, while the inferior parathyroid glands sit more variable and often slightly inferior, tending toward the lower pole and thyrothymic ligament. The recurrent laryngeal nerve ascends in the tracheoesophageal groove deep and medial to the thyroid lobe, and the inferior thyroid artery approaches the posterior aspect of the gland where it commonly crosses near parathyroid tissue. Small organs. Big consequences. A lateral assessment matters because parathyroid location is the difference between a routine thyroidectomy and a postoperative hypocalcemia case; devascularization or inadvertent excision of these epithelial bodies is a common mechanism of transient or permanent hypoparathyroidism. This angle also supports teaching the embryologic explanation for variability, with inferior glands (from the third pharyngeal pouch) migrating farther and showing ectopic positions in the mediastinum or within the thyroid parenchyma, while superior glands (fourth pouch) cluster more reliably on the dorsal thyroid.
