Seen in lateral profile, the thyroid gland (glandula thyroidea) sits anterolateral to the upper trachea and inferior to the larynx, with the isthmus crossing the anterior midline and the right and left lobes extending superiorly toward the thyroid cartilage and inferiorly toward the lower cervical tracheal rings. The posteromedial border of each lobe approximates the tracheoesophageal groove, where the recurrent laryngeal nerve ascends, while the lateral surface faces the carotid sheath region. Superior and inferior poles taper toward their vascular pedicles, the superior pole nearer the external branch of the superior laryngeal nerve and the inferior pole nearer the typical entry of the inferior thyroid artery. Clear topography. A lateral depiction matters because it clarifies what lies immediately deep to the strap muscles and where the gland wraps the airway, an orientation that drives safe planes in thyroidectomy and hemithyroidectomy. The relationship of the posterior lobe to the tracheoesophageal groove underpins two high-stakes complications: recurrent laryngeal nerve injury with postoperative dysphonia, and parathyroid devascularization, since the superior and inferior parathyroid glands often nestle on the posterior aspect near the inferior thyroid artery. Goiter, thyroiditis, and large nodules alter this same geometry, displacing the trachea and changing the apparent position of the isthmus and poles on physical exam and cross-sectional imaging.
