Description
Posteriorly, the right and left lobes of the glandula thyroidea flank the upper trachea and converge across the midline at the isthmus, which lies inferior to the laryngeal framework. Along the medial aspect of each lobe, the tracheoesophageal groove forms a narrow corridor where the recurrent laryngeal nerves ascend toward the larynx, running deep and close to the posterior thyroid capsule. Superiorly, the lobes approach the cricoid cartilage region; inferiorly, they taper toward the thoracic inlet, maintaining a paired, symmetric relationship around the airway. Tight anatomy. This posterior perspective matters because it mirrors the surgeon’s problem set during thyroidectomy: protecting the recurrent laryngeal nerve while ligating posterior thyroid vessels and mobilizing the gland off the trachea and esophagus.
