Description
Seen in lateral profile, the thyroid gland (glandula thyroidea) presents a convex external contour with a superior pole pointing cranially and a broader inferior pole extending caudally toward the thoracic inlet. The lateral surface of the ipsilateral lobe lies superficial to the trachea and more medial than the carotid sheath, while its posteromedial aspect relates to the larynx and upper tracheal rings. Arterial inflow is suggested along the superior margin from the superior thyroid artery and along the inferior pole from branches of the inferior thyroid artery, with venous channels coursing anteroinferiorly toward the superior, middle, and inferior thyroid veins. Proximity to the posterior border implies the usual course of the recurrent laryngeal nerve in the tracheoesophageal groove and the typical location of the parathyroid glands on the posterior surface. For operative planning, this lateral morphology is the viewpoint that clarifies why the superior thyroid vessels demand careful ligation close to the gland to protect the external branch of the superior laryngeal nerve near the superior pole. Goiter and thyroid nodules commonly expand the lobe laterally and inferiorly, narrowing the tracheal lumen and distorting the relationship to the carotid sheath, findings mirrored on ultrasound and CT when the gland’s outer contour and vascularity are assessed. A clean lateral outline also reinforces teaching of the gland’s capsular planes and the anatomic basis of postoperative dysphonia from recurrent laryngeal nerve injury.
