Description
Lateral neck anatomy is centered on the common carotid artery ascending within the carotid sheath and dividing at the level of the superior border of the thyroid cartilage into the internal and external carotid arteries. At the posteromedial aspect of this bifurcation sits the carotid body (glomus), with the carotid sinus expanding along the proximal internal carotid artery. The carotid body nerve, most often a branch of the glossopharyngeal nerve (Hering’s nerve), takes a short, direct course to the chemoreceptor tissue at the bifurcation, running in close proximity to the arterial adventitia and small feeding vessels. Small but specific anatomy. Physiologically, this is the afferent limb of the peripheral chemoreceptor reflex, carrying hypoxia and hypercapnia signals to the nucleus tractus solitarius and driving ventilatory and autonomic responses. Clinically, the relationship between the carotid body nerve and the carotid bifurcation matters during carotid endarterectomy and carotid artery stenting, where dissection or retraction can irritate regional baroreceptor and chemoreceptor afferents and contribute to perioperative bradycardia or blood pressure lability. It also frames the surgical and imaging workup of carotid body paraganglioma, a hypervascular mass at the bifurcation that can encase the internal and external carotid arteries and sit adjacent to cranial nerves IX, X, XI, and XII.
