Description
Seen in lateral profile on the left neck, the common carotid artery ascends within the carotid sheath, anterior to the prevertebral musculature and medial to the internal jugular vein as it tracks superiorly toward the level of the thyroid cartilage. Near its bifurcation, the vessel divides into the internal carotid artery, continuing posterosuperiorly without cervical branches, and the external carotid artery, which courses more anterosuperiorly toward the face and scalp. The trachea and laryngeal airway lie anterior and slightly medial in this orientation, providing a familiar midline reference against the vascular column. Understanding this side-on relationship is central to neck procedures because the carotid pulse is palpated against deeper structures, and the internal jugular vein sits lateral to the artery in most individuals, a relationship targeted during ultrasound-guided central venous access. Carotid atherosclerosis commonly concentrates at the bifurcation and proximal internal carotid, where turbulent flow and plaque can lead to transient ischemic attack or stroke, and where carotid endarterectomy must protect the hypoglossal nerve and adjacent vagus within the sheath. Landmarks matter here. Small deviations can have consequences. Use this illustration in gross anatomy and head and neck courses to teach the carotid sheath contents and the internal versus external carotid branching logic, and in procedural training materials for jugular cannulation, carotid endarterectomy, and cross-sectional imaging orientation for CTA/MRA correlation.
