Posteriorly, the paired common carotid arteries ascend within the carotid sheath on either side of the cervical vertebral column, running superiorly toward the level of the upper thyroid cartilage where each will bifurcate into the internal and external carotid arteries. Medial to them, the trachea and esophagus lie in the visceral compartment, while the internal jugular veins course more laterally in the sheath. Deep cervical muscles and the prevertebral layer sit immediately anterior to the cervical vertebrae and spinal cord, placing the great vessels in clear relation to the posterior neck framework. Orientation from behind clarifies how the common carotid relates to structures that shape surgical corridors and complication patterns.
