Description
Posterior to the thyroid lobes, the superior and inferior parathyroid glands (glandulae parathyroideae) sit as small ovoid epithelial bodies against the thyroid capsule, classically two per side. The superior parathyroids typically lie more constant in position near the posterior aspect of the upper thyroid pole, while the inferior parathyroids occupy a more variable location closer to the lower pole and the thyrothymic tract. A shallow groove between the thyroid and the tracheoesophageal region frames their medial relationship, with the carotid sheath remaining lateral and the cervical midline structures medial. Small vessels approach the glands from the posterolateral direction in keeping with their usual supply from branches of the inferior thyroid artery. Posterior spatial orientation matters because parathyroid preservation is a defining technical goal in thyroidectomy and central neck dissection, where capsular dissection along the posterior thyroid surface can keep parathyroid tissue and its end-arterial blood supply intact. This is also the surgical neighborhood of the recurrent laryngeal nerve in the tracheoesophageal groove, and the parathyroids often act as practical landmarks when teaching how to avoid nerve traction, thermal injury, or inadvertent ligation of feeding vessels. Ectopic or displaced inferior glands can complicate reoperative parathyroidectomy for primary hyperparathyroidism.
