Seen from a lateral (peripheral) viewpoint on the left side of the heart, thin chordae tendineae spuriae traverse the left ventricular lumen as irregular tendinous strings distinct from the primary mitral valve chordae. Strands course obliquely between the ventricular wall and trabeculae carneae, often spanning from the interventricular septum toward the lateral free wall and lying inferior to the mitral annulus and valve leaflets. Their relationship to papillary muscles is variable, with attachments that may appear accessory rather than inserting cleanly on the anterior or posterior mitral leaflet. Fine, filamentous anatomy. This view matters because chordae tendineae spuriae can be mistaken for ruptured chordae, fibrin strands, or intracavitary thrombus on echocardiography, and their recognition reduces false-positive interpretations of mitral pathology.
