Description
Seen from a left lateral aspect, the left ventricle is opened to reveal papillary muscles projecting from the ventricular wall and a dense web of chordae tendineae within the ventricular cavity. Among the primary and secondary chordae that course superiorly toward the mitral valve leaflets, multiple chordae tendineae spuriae traverse between papillary muscles, trabeculae carneae, and the ventricular endocardium without inserting on the free margins of the cusps. These thin fibrous strings run obliquely and transversely, forming internal bridges inferior to the mitral annulus and anterior to the posteromedial ventricular wall. Spurious chordae are a familiar source of confusion in dissection and echocardiography because they can mimic ruptured chordae or vegetations when they appear as mobile linear echoes in the left ventricle. They also matter in operative planning: during mitral valve repair or papillary muscle relocation, distinguishing true leaflet-supporting chordae from chordae spuriae helps preserve the subvalvular apparatus while avoiding unnecessary division of innocuous intracavitary bands. Small structure, common pitfall. Medical educators can use this lateral ventricular view in cardiac anatomy teaching to contrast functional chordae tendineae with non-valvular tendinous bands, and in pathology modules covering mitral regurgitation, chordal rupture, and endocarditis differentials on transthoracic or transesophageal echo. It also fits well in surgical atlases illustrating exposure of the left ventricle and subvalvular apparatus during mitral procedures or ventricular reconstruction.
