Description
Positioned on the left (sinister) side of the heart, the left ventricle is presented in lateral aspect, with its thick myocardium forming the dominant inferolateral contour and the ventricular cavity tapering toward the apex. Superiorly and medially, the mitral valve orifice opens into the ventricle, while the aortic valve orifice sits more anterior and superior in the left ventricular outflow tract. Chordae tendineae descend from the mitral leaflets to the anterolateral and posteromedial papillary muscles, spanning the ventricular lumen in a fan-like arrangement. Lateral visualization makes the aortomitral relationship easy to teach because the fibrous continuity between the anterior mitral leaflet and the aortic root becomes intelligible as a single functional unit. Surgeons exploit this anatomy during mitral valve repair, where preservation of chordae tendineae helps maintain left ventricular geometry and reduces postoperative dysfunction, and during aortic valve or root procedures where the proximity of the mitral apparatus can complicate exposure. It also maps directly onto pathophysiology: ischemia affecting the posteromedial papillary muscle (often with right coronary or circumflex territory infarction, depending on dominance) predisposes to acute mitral regurgitation.
