Description
Viewed from the left lateral aspect, the left ventricular inlet appears as a broad atrioventricular orifice between the left atrium superiorly and the left ventricle inferiorly. The mitral valve (valva atrioventricularis sinistra) forms the boundary, with its anterior leaflet oriented toward the left ventricular outflow tract and aortic root, and its posterior leaflet positioned more inferolaterally along the mural side of the annulus. Along the ventricular surface, chordae tendineae course from the free margins of both leaflets to the anterolateral and posteromedial papillary muscles, which rise from the compact myocardium of the left ventricular wall. This lateral perspective matters when you need to teach or plan around the geometry of the mitral apparatus and its relationship to the left ventricular cavity. Mitral regurgitation from leaflet prolapse or chordal rupture, and functional regurgitation from annular dilatation with papillary muscle displacement, both make more sense when you can track leaflet coaptation from annulus to papillary muscles in a single line of sight. Surgical concepts such as annuloplasty sizing, chordal replacement, and the risk of systolic anterior motion (SAM) toward the left ventricular outflow tract are easier to explain with the anterior leaflet clearly placed adjacent to the outflow region.
