Description
Viewed from the left lateral aspect, the trabecular portion of the left ventricle is defined by prominent trabeculae carneae projecting from the endocardial surface into the ventricular cavity. These interlacing muscular ridges course in multiple directions, creating a coarse spongy relief that contrasts with the smoother outflow tract toward the aortic vestibule. More superiorly and medially, the trabeculae converge toward the bases of the papillary muscles, which rise from the ventricular wall and orient toward the mitral valve apparatus. A thick compact myocardium forms the external contour lateral to the trabeculated zone. This lateral angle matters when teaching how left ventricular architecture supports efficient systolic ejection while protecting the inflow apparatus from leaflet prolapse. Papillary muscle dysfunction after ischemia, often involving the posteromedial papillary muscle supplied by a single coronary territory, can precipitate acute mitral regurgitation, and understanding its relationship to surrounding trabeculae helps explain why rupture can be catastrophic. The same anatomy underpins the echocardiographic and cardiac MRI distinction between normal trabeculation and left ventricular noncompaction, where excessive trabeculae and deep recesses can mimic thrombus or alter measured chamber volumes.
