Description
Presented in section, the left ventricle is opened to emphasize the trabecular part (trabeculated zone) with prominent trabeculae carneae projecting from the endocardial surface. Coarse muscular ridges course along the lateral wall and sweep inferiorly toward the apex, separated by deep intertrabecular recesses, while the thicker compact myocardium lies more external to this mesh-like inner layer. The cavity contours suggest continuity with the inflow and outflow portions of the chamber even if the mitral apparatus and aortic vestibule are only partially included. Orientation is effectively lateral in profile. Left ventricular trabeculation is a frequent teaching point when distinguishing normal anatomical variation from disease, because the degree and pattern of trabeculae carneae can mimic or meet criteria used in evaluating left ventricular noncompaction cardiomyopathy. That distinction has real downstream consequences in echocardiography and cardiac MRI reporting, where the noncompacted to compacted myocardial ratio is measured in specific planes and at defined phases of the cardiac cycle. A clean sectional view also helps clarify why thrombus can lodge within recesses after anterior myocardial infarction and regional akinesia, and how altered flow patterns arise in dilated cardiomyopathy.
