Description
Within the left ventricle, the endocardial surface is rendered as a trabeculated zone dominated by trabeculae carneae arranged in irregular muscular ridges and bridges along the ventricular wall. Toward the apex these bundles become denser and more interlacing, while the more superior portion transitions toward a smoother outflow region approaching the aortic vestibule. The lateral wall is emphasized, with trabecular bands coursing obliquely and intersecting in a pattern that highlights the relief of the ventricular lining. Recognition of this configuration matters when you need to separate normal trabeculation from pathology. Prominent left ventricular trabeculae can mimic mural thrombus or endocardial vegetation on echocardiography and cardiac CT, and the apical emphasis of trabeculation is a common source of interpretive error. The same anatomic territory is central to discussions of left ventricular noncompaction, where excessive trabeculation and deep intertrabecular recesses are assessed against diagnostic thresholds, and to ventricular arrhythmia substrates that often cluster in the apical and lateral segments.
