Description
Seen from the left lateral aspect, fine chordae tendineae spuriae traverse the left ventricular cavity, coursing obliquely from the ventricular wall toward trabeculae carneae rather than inserting on the mitral valve leaflets. Their threadlike strands lie deep to the endocardial surface, suspended within the lumen inferior to the mitral annulus and anterior to the papillary muscle tips when those landmarks are included. Relations are best appreciated against the thicker muscular ridges of the interventricular septum medially and the left ventricular free wall laterally. Small structures. Clear orientation. Spurious chordae matter because they can be mistaken for pathologic strands on echocardiography or cardiac MRI, and they sometimes contribute to flow-related artifacts within the left ventricle. In the electrophysiology setting, a left ventricular false tendon may contain Purkinje fibers and has been associated with idiopathic ventricular ectopy, a point that comes up when correlating intracardiac mapping with anatomic substrates. For surgeons and anatomists, their non-valvular attachments also help reinforce the distinction between true chordae (to mitral leaflets) and accessory fibrous bands spanning the ventricular lumen. Cardiac anatomy courses and cardiovascular pathology lectures use this lateral view to teach normal anatomic variants and to prevent overcalling a thrombus, vegetation, or ruptured chord on ultrasound. It also fits well in imaging atlases, echo reporting guides, and device-procedure training materials where spatial relationships within the left ventricle must be communicated quickly and accurately.
