Lingule Attached to the Left Lung

Description

Arising from the anterior aspect of the left lung’s superior lobe, the lingula (lingula pulmonis) projects inferiorly as a tongue-like extension that lies medial to the costal surface and approaches the cardiac notch. Its free margin forms part of the anteroinferior contour of the upper lobe, positioned superior to the dome of the left hemidiaphragm and anterior to the hilum. Medially, the lingula drapes toward the pericardium, while laterally it remains bounded by the pleural reflections along the anterior thoracic wall. Clinically, the lingula matters because it behaves like the right middle lobe in both ventilation patterns and disease distribution, and it is a frequent site of dependent atelectasis or mucus plugging in patients who are supine or have impaired clearance. The close relationship to the heart and pericardium also explains why left anterior lung pathology can silhouette the left heart border on a frontal chest radiograph, and why lingular consolidation can mimic cardiac enlargement at a glance.
