Description
Arising from the anteroinferior portion of the left superior lobe, the lingula forms a tongue-like projection that drapes over the left cardiac border. Its superior and inferior lingular segments lie anterior to the hilum and are bounded posteriorly by the oblique fissure as the lung slopes toward the lower lobe. Medially, the visceral pleura of the lingula approaches the pericardium, while laterally it conforms to the inner surface of the thoracic wall and costal pleura. Clear laterality. Clinically, the lingula behaves like the right middle lobe in both anatomy and disease patterns, because its relatively narrow bronchial pathway and dependent position in supine patients can predispose it to atelectasis, mucus plugging, and recurrent infection. Radiologists and pulmonologists often localize lingular pneumonia or scarring on frontal chest radiographs by loss of the left heart border silhouette, then confirm distribution on CT in relation to the oblique fissure and segmental bronchi. Bronchoscopy planning also benefits from a mental map of the superior versus inferior lingular segment when targeting lavage or biopsy.
