Description
From the tracheal bifurcation at the carina, the right and left principal bronchi descend into their respective hila, with the right main bronchus taking a shorter, wider, more vertical course than the left. Each principal bronchus gives rise to lobar (secondary) bronchi that pass inferiorly and laterally into the lung substance, then divide into segmental (tertiary) bronchi that define the bronchopulmonary segments. Distally, the airway caliber narrows through smaller conducting branches toward the terminal and respiratory bronchioles, which lead into alveolar ducts and alveoli in the peripheral parenchyma. Orientation cues typically include the more oblique left main bronchus crossing toward the left hilum as it passes anterior to the esophagus and thoracic aorta region, while the right side aligns more directly with the tracheal axis. Clear asymmetry. That right sided geometry matters in practice. Aspirated foreign bodies and vomitus most often track into the right main bronchus and then into the bronchus intermedius toward the lower lobe, a pattern that also explains common dependent pneumonias in supine patients. Segmental branching is the surgical map: segmentectomy and bronchoscopy depend on identifying segmental bronchi and their relationship to lobar bronchi, while lymph node stations cluster around the main and lobar bronchi near the carina and hila. The carina itself remains a key bronchoscopic landmark, and its blunting can signal subcarinal adenopathy or central tumor.
