Description
Oriented within the thorax, the trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, each entering its lung at the hilum with the pulmonary arteries and veins. The right main bronchus typically courses more vertical and wider than the left, then divides into lobar bronchi supplying the superior, middle, and inferior lobes, while the left main bronchus passes inferolateral beneath the aortic arch before branching to the superior and inferior lobes. Segmental bronchi extend distally toward the bronchopulmonary segments, transitioning to bronchioles that terminate in alveolar ducts and alveoli within the peripheral lung parenchyma. Spatially, the bronchial tree fans laterally from the mediastinum, with progressively smaller airways running anterior, posterior, and inferior through each lobe. Orientation matters because airway caliber and angles determine where inhaled material goes. Aspirated food, gastric contents, or a misplaced endotracheal tube most often enters the right bronchial system, commonly the right lower lobe bronchus, reflecting the more in-line relationship between the trachea and right main bronchus. Bronchial anatomy also sets the map for bronchoscopy and for segmental resection in lung cancer, where surgeons follow segmental bronchi and accompanying arteries to preserve adjacent bronchopulmonary segments.
