Framed from a posterior perspective, the adrenal (suprarenal) gland sits capping the superomedial pole of the kidney, with attention drawn to its medial margin where the gland angles toward the great vessels. The superior pole lies closer to the diaphragm, while the inferior aspect approaches the renal hilum; laterally the gland tapers toward the kidney’s upper pole and medially it borders the periaortic and paracaval space. Expect adjacent retroperitoneal landmarks to be implied even if not fully rendered, including the diaphragmatic crus posteriorly and the renal fascia enveloping the upper kidney. The medial border is a practical landmark in adrenal surgery because it is where vascular control quickly becomes the problem to solve. On the right, the short right suprarenal vein drains directly into the inferior vena cava and is commonly encountered near the medial aspect, making this margin the site where avulsion bleeding can occur during mobilization. On the left, the medial relation shifts toward the aorta and the left suprarenal vein typically joins the left renal vein, and this view helps teach why left adrenalectomy often involves more medial dissection along the renal vein and diaphragmatic crus.
