Uterine Boundary

Description

Lateral borders of the uterus are presented with the fundus superiorly and the cervix inferiorly, emphasizing the margo lateralis (lateral uterine margin) where the uterine tube meets the uterine cornu. Along this superior lateral margin, the mesosalpinx drapes around the uterine tube while the mesometrium extends inferiorly as the broad ligament attachment. Close by, the ligamentum teres uteri (round ligament) runs anterolaterally from the uterine horn toward the deep inguinal ring, and the ligamentum ovarii proprium (ovarian ligament) courses posterolaterally toward the ovary. Understanding the uterine boundary at the lateral margin matters because this is where adnexal anatomy and uterine support converge, and where surgeons routinely work when mobilizing the uterus. During hysterectomy, the relationship of the uterine cornu to the fallopian tube and broad ligament guides safe entry into avascular planes, while the nearby uterine vessels in the base of the broad ligament and the ureter passing inferior to the uterine artery set up the classic risk point at the level of the cervix. Cornual ectopic pregnancy and proximal tubal disease also localize to this junction. A tight neighborhood.
