Arising from the superolateral uterine angle near the uterine tube, the round ligament of the uterus (ligamentum teres uteri) is seen leaving the lateral uterine border (margo lateralis uteri) and coursing anterolaterally within the broad ligament toward the deep inguinal ring. The lateral margin of the uterine body transitions inferiorly toward the cervix, where the uterine artery typically approaches the uterus within the cardinal ligament (transverse cervical ligament) at the level of the internal os. Medially lies the endometrial cavity contained by the myometrium, while laterally the peritoneal reflections define the mesometrium and the uterine adnexal attachments. Clean anatomy. This lateral-border emphasis matters when you need to teach or plan around the ligamentous and vascular relationships at the uterine cornu. During hysterectomy or uterine artery ligation, the uterine artery crosses superior to the ureter near the cervix (water under the bridge), and traction on the uterus can shift these relationships, raising the risk of ureteric injury. The round ligament also provides a practical landmark in laparoscopy for entering the broad ligament and orienting the surgeon to the anterior leaf and vesicouterine peritoneal fold.
