Description

Along the lateral margins (margo lateralis) of the uterus, the uterine borders are defined by the contour of the corpus uteri tapering inferiorly into the isthmus and cervix, with the fundus forming the superior dome between the uterine horns. The uterine vessels track in a sinuous ascending course along each lateral edge, approaching the uterus from the broad ligament and running superiorly toward the cornual region. Medially lies the endometrial cavity silhouette within the myometrium, while laterally the border implies the attachment line of the perimetrium and the mesometrium of the broad ligament. Orientation is bilateral and symmetric. Clear borders matter. That lateral border is more than a shape cue, it is the operative corridor where the uterine artery approaches the cervix and crosses superior to the ureter near the level of the internal os, the classic water under the bridge relationship that drives ureteral injury risk in hysterectomy and uterine artery ligation. The winding appearance of the ascending vessels also matches the physiologic tortuosity that accommodates uterine enlargement in pregnancy and helps explain brisk bleeding when the vessel is transected close to the uterine wall. For teaching, it is a clean way to separate uterine contour anatomy from the surrounding adnexal structures.
