Description
Prominent on the posterior surface of the male liver, the bare area (area nuda hepatis, pars affixa) forms a broad triangular zone devoid of peritoneum where hepatic parenchyma lies directly against the inferior surface of the diaphragm. Its margins are defined by reflections of visceral peritoneum that create the anterior and posterior layers of the coronary ligament, which converge laterally into the right and left triangular ligaments. Medially and inferiorly, the nonperitoneal region sits adjacent to the groove for the inferior vena cava, with the right lobe extending laterally and superiorly toward the diaphragmatic dome. A true nonperitoneal interface. This posterior, nonperitoneal contact explains why disease does not always respect classic peritoneal compartments. Subphrenic abscess, hepatic hydatid disease, or a posterior right-lobe tumor can adhere to the diaphragm at the bare area and track inflammation or invasion toward the thorax, occasionally presenting with pleuritic symptoms or a reactive pleural effusion rather than abdominal guarding. During mobilization of the right lobe in open or laparoscopic hepatectomy, surgeons dissect along the coronary ligament to free the pars affixa while staying oriented to the retrohepatic inferior vena cava, a step where bleeding can be brisk and spatial landmarks matter. Faculty can drop this into gross anatomy teaching on peritoneal reflections, or into hepatobiliary surgery and radiology content to clarify why posterior hepatic lesions behave differently on cross sectional imaging and at operation.
