Jejunum Posterior Orientation

Description

Posteriorly oriented anatomy of the proximal small intestine centers on the jejunum as it lies predominantly in the left upper and central abdomen, suspended by the mesentery of the small intestine (mesenterium) from the posterior abdominal wall. Mesenteric sheets converge along the mesenteric root, an oblique attachment that runs from the region of the duodenojejunal flexure down toward the ileocecal junction, placing the jejunal loops more superior and left of the distal ileum. Jejunal mesentery carries the superior mesenteric artery and vein within its fat and connective tissue, with jejunal arterial branches coursing distally into the bowel wall and the mesenteric border forming the medial attachment line of each loop. Surgical orientation to the posterior mesenteric attachment matters because the root of the mesentery crosses key retroperitoneal structures, and mobilization during laparotomy or laparoscopy risks traction or injury to superior mesenteric vessels. Internal hernias through mesenteric defects, including those created after Roux-en-Y reconstruction, commonly entrap proximal small bowel, and the posterior relationship of bowel loops to the mesenteric root helps explain the closed-loop obstruction pattern. Ischemia can follow. This perspective also clarifies why proximal jejunum tends to show fewer arterial arcades and longer vasa recta than ileum, a point used in gross anatomy and operative identification.
