Description
Viewed laterally, the male ileum forms a mobile distal segment of the small intestine (intestinum tenue) suspended from the posterior abdominal wall by its mesentery. Proximally it continues from the jejunum, then courses inferiorly and medially within the lower abdomen and pelvis before turning toward the right iliac fossa. The terminal ileum (pars terminalis) approaches the caecum and ileocaecal junction, where the bowel contour often appears more uniform from fewer plicae circulares, while aggregated lymphatic nodules (Peyer patches) cluster along the antimesenteric border. A lateral surface contour view is a practical teaching angle because it matches how the distal small bowel is encountered in the right lower quadrant during appendicitis workups and ileocaecal surgery. Subtle changes in terminal ileum caliber and wall profile help clinicians localize disease to the distal bowel, where Crohn disease commonly produces segmental thickening, creeping fat along the mesenteric attachment, and a fixed, narrowed terminal segment near the ileocaecal valve. Orientation matters. Confusing ileum with jejunum is a frequent early error.
