Description
Curving in a characteristic C-shape, the male duodenum is shown as the first segment of the small intestine, continuous proximally with the pylorus of the stomach and distally with the jejunum at the duodenojejunal flexure. The superior part (pars superior) runs posterosuperiorly from the pyloric canal before turning inferiorly into the descending part (pars descendens), which lies to the right of the midline and drops toward the level of the head of the pancreas. From there, the horizontal part (pars horizontalis) crosses medially, and the ascending part (pars ascendens) climbs superiorly to the left to meet the jejunum. Close apposition to the pancreatic head and the expected course of the common bile duct along the posteromedial aspect of the second part are implied by the duodenum’s fixed, retroperitoneal position. This configuration matters because the duodenum frames several high-yield clinical junctions: the hepatopancreatic ampulla typically opens on the major duodenal papilla in the second part, a focal point for choledocholithiasis and gallstone pancreatitis. Its posterior relations also explain why a posterior duodenal bulb ulcer can erode the gastroduodenal artery and produce brisk upper gastrointestinal hemorrhage. Short. Fixed. Unforgiving.
