Description
Seen from a medial viewpoint, the left lung is oriented to emphasize the cardiac notch (incisura cardiaca) along the anterior border, where the pulmonary parenchyma curves around the heart. Superior to this indentation, the apex and superior lobe contour toward the thoracic inlet, while inferiorly the concave diaphragmatic surface meets the sharp inferior border. Medially, the hilum region would be expected just posterior to the notch, with the root of the lung positioned for the passage of the left main bronchus and pulmonary vessels. Appreciating the cardiac notch matters whenever you teach or interpret the left lung’s asymmetry compared with the right, and it is the anatomic basis for the lingula projecting anteroinferiorly as a tongue-like extension of the superior lobe. On chest radiography and CT, this anterior indentation and the adjacent cardiophrenic angle help orient the heart-lung interface, and in the operating room it frames the anterior approach during left-sided thoracic procedures where the pericardium and phrenic nerve lie in close proximity.
