Curving across the convexity of the cerebral hemisphere, the superior anastomotic vein of Trolard tracks over the parietal lobe to link the superficial middle cerebral venous territory laterally with the superior sagittal sinus along the midline. Adjacent sulci and gyri of the frontoparietal region, including the central sulcus area, provide the surface landmarks that orient its superior course and its relationship to the perirolandic cortex. Red cortical arteries and blue superficial cortical veins form a dense pial network around it, while the venous channel itself remains superficial to the cerebral cortex and deep to the arachnoid plane. A prominent bridging segment is implied where it approaches the superior sagittal sinus. Classic Trolard anatomy. This vein matters because it is a major collateral pathway in superficial cerebral venous drainage, and its size and dominance vary inversely with the vein of Labbé, a pattern that influences both venous infarct risk and surgical planning. During parasagittal craniotomy or exposure near the superior sagittal sinus, injury to a dominant Trolard vein or its bridging segment can precipitate postoperative venous congestion and hemorrhagic infarction in the parietal or perirolandic region.
