Superior Anastomotic Vein of Trolard

Dominating the parasagittal convexity, the superior anastomotic vein (vein of Trolard) courses superiorly over the parietal lobe to bridge the superficial middle cerebral vein territory laterally with the superior sagittal sinus medially. Cortical veins from the precentral and postcentral regions converge into this channel as it tracks across the superior parietal lobule, superficial to the cerebral cortex and deep to the arachnoid. Parasagittally, its termination lies just lateral to the falx cerebri, where the superior sagittal sinus runs along the midline. Color separation of arteries and veins supports quick discrimination of venous collectors from adjacent arterial branches along the cortical surface. A key landmark. The Trolard vein is one of the principal superficial anastomotic pathways, along with the vein of Labbé, and this view clarifies how venous drainage can reroute when a sinus or cortical vein thromboses. Neurosurgeons planning a frontoparietal craniotomy or an interhemispheric approach pay close attention to this vessel because injury during dural opening or cortical exposure can precipitate venous infarction and hemorrhagic conversion in the perirolandic cortex. Venous anatomy also explains the distribution of parasagittal edema and seizures seen in superior sagittal sinus thrombosis.
