Description

Superiorly, the apex of the adult female urinary bladder tapers into a subtle anterior point where the median umbilical ligament (urachus remnant) courses toward the umbilicus. Just inferior to this apex, the dome (superior surface) expands posteriorly, while the peritoneal reflection drapes over it to form the vesicouterine pouch between bladder and uterus. Anterior relations include the retropubic space (space of Retzius) deep to the pubic symphysis, with the bladder wall sitting posterior to the pubic bones and anterior to the uterus and cervix. This superior bladder anatomy matters when you need clean orientation between intraperitoneal and extraperitoneal territory. A distended bladder elevates the peritoneal reflection, and that shift underlies suprapubic catheter placement and open cystostomy technique, where staying extraperitoneal reduces the risk of bowel injury and urinary peritonitis. The urachal tract is also a real-world landmark: persistent urachus can present with umbilical urine leakage, and urachal carcinoma tends to arise near the bladder dome and track toward the umbilicus. Use this plate in pelvic anatomy teaching (gross anatomy, urogynecology, and surgical anatomy modules) to anchor discussions of peritoneal pouches, bladder distension, and safe access routes above the pubic symphysis. It also fits well in urology and radiology publications that need a clear reference for dome-level lesions and urachal pathology correlations. Anatomical accuracy verified by SciePro's Medical Advisory Board.
