Description

Centered in the anterior pelvis, the superior portion (apex) of the adult female urinary bladder is shown tapering anteriorly toward the anterior abdominal wall, where the median umbilical ligament (urachus remnant) continues to the umbilicus. The dome (vertex) lies superior to the bladder body, with peritoneum draping over its superior surface and reflecting onto adjacent pelvic structures; anteriorly it approaches the retropubic space (space of Retzius) and posteriorly it transitions toward the bladder base and trigone region. Medial continuity of the bladder wall layers is implied at the apex, where the detrusor forms the muscular coat beneath the serosa/adventitia. A clear midline landmark. Surgeons care about the bladder apex because it is the common site of urachal anomalies in adults, including urachal cysts, sinus tracts, and, rarely, urachal adenocarcinoma presenting with suprapubic pain, recurrent infection, or hematuria. The superior bladder dome is also the most vulnerable region during trocar entry and low midline incisions when the bladder is distended, and it becomes a target during suprapubic cystostomy placement where accurate midline orientation helps avoid peritoneal violation. Obstetric and gynecologic procedures benefit from this view when teaching how bladder filling changes superior displacement and peritoneal reflections.
