Centered on the superior anterior urinary bladder, the apex (vertex vesicae) tapers toward the anterior abdominal wall and continues as the median umbilical ligament, the fibrous remnant of the urachus. The dome of the bladder lies superior to the trigone and neck and sits posterior to the pubic symphysis, separated by the retropubic space, while the urethra exits inferiorly. In the adult female, the posterior bladder surface is closely related to the anterior wall of the vagina and the inferior uterus or cervix, relationships that become more apparent as the bladder fills and the peritoneal reflection shifts over the superior surface. This focal emphasis on the bladder apex matters because it is the landmark region for urachal remnants and their complications, including urachal cyst, sinus, and (rarely) adenocarcinoma arising along the midline tract to the umbilicus.
