Description
Centered at the inferior aspect of the adult female urinary bladder, the bladder neck (collum vesicae) narrows into the internal urethral orifice and continues as the proximal urethra. Posteriorly, the trigone slopes toward the vesical outlet, while the detrusor muscle fibers thicken circumferentially at the vesicourethral junction. Inferior support is implied by the intimate relationship to the pelvic floor, with the bladder base positioned anterior to the vagina and cervix in typical female pelvic anatomy. A compact transition zone. Bladder neck anatomy matters because continence and voiding both hinge on this short segment where detrusor contraction must coordinate with urethral relaxation. Surgical and iatrogenic problems often localize here: bladder neck obstruction can follow anti-incontinence procedures, and periurethral scarring may distort the internal urethral meatus, producing a high-pressure, low-flow pattern on urodynamics. In radical hysterectomy or anterior vaginal repair, dissection planes near the vesicourethral junction risk denervation that contributes to postoperative urinary retention and urgency.
