Description
Posterior to the left hilum, the left inferior lobar bronchus (bronchus lobaris inferior sinister) descends toward the lower lobe, giving off basilar segmental bronchi that course inferiorly and laterally toward the diaphragmatic surface. A dorsal perspective typically places the bronchial lumen and branching pattern in the foreground, with the pulmonary artery lying superior to the bronchus at the root and the inferior pulmonary vein occupying a more anteroinferior position before it enters the left atrium. More peripherally, the bronchial tree tracks within the posterior basal parenchyma, aligned with the pyramid-like geometry of the lower lobe and its basilar segments. Posterior airway anatomy matters when you are correlating bronchoscopy, CT (axial and coronal reformats), and surgical planes in the left lower lobe, where segmental anatomy guides both resection margins and lymph node sampling. The left inferior lobar bronchus is also a practical landmark when teaching the common distribution of aspiration and post-obstructive infection, and when explaining why mucus plugging in dependent basilar segments can be stubborn in supine patients. Small relationships at the hilum, especially bronchus-to-vessel orientation, drive safe dissection during a left lower lobectomy or segmentectomy. Orientation is the point.
