Description
Curving within the male auricle, the antihelix (anthelix) rises as a Y-shaped cartilaginous ridge just medial and posterior to the helix, separating the scapha laterally from the conchal bowl medially. Its superior and inferior crura frame the triangular fossa superiorly and blend inferiorly toward the concha, where the cavum and cymba conchae lead into the external acoustic meatus. Anteriorly, the tragus and the root of the helix sit at the auricular inlet, while the lobule remains inferior and noncartilaginous. Subtle surface relief matters here. Auricular cartilage landmarks are the practical map for otoplasty planning and for documenting congenital variation such as prominent ear, where inadequate antihelical folding or a deep conchal bowl increases auriculocephalic protrusion. This view supports teaching how the antihelix defines normal auricular topography and why surgeons recreate an antihelical fold with mattress sutures (Mustarde technique) or reduce conchal excess (Furnas setback) while preserving the conchal contour at the external auditory canal. It also helps orient surface anesthesia, where the auriculotemporal nerve supplies anterior-superior auricle and the great auricular nerve supplies much of the lobule and posterior-inferior pinna.
